Appearance Release

I agree that Nordic Walk Now and Suzanne M. Nottingham, Inc. may produce, distribute, exhibit, broadcast, advertise, and/or use my appearance or any portion thereof including photographs of me taken during my training/program/event by any media throughout the world in perpetuity, for any purpose. You and your assigns may use my name, voice, and likeness in connection therewith. I further agree that Nordic Walk Now and Suzanne M. Nottingham, Inc. may change, edit, add to, or subtract from the material featuring me, but shall not be obligated to utilize any of the photographed materials of me or of my participation at Nordic Walk Now events. I waive any and all claims or demands of any nature, which I may now have or may thereafter, have against Nordic Walk Now or Suzanne M. Nottingham, Inc. and/or against any person, firm, or corporation to whom you may sell, license, assign, or transfer your rights in the material and/ or uses of my name, voice and likeness, in connection with any such use.

Liability Release

I am aware of, understand, and take full responsibility for the fact that using any equipment to enhance physical fitness, indoors and outside, including uses of Nordic walking poles, may cause bodily injury to myself or others. I do hereby release, forever discharge, and agree to hold harmless Suzanne M. Nottingham, Inc., Nordic Walk Now, as well as its directors, instructors, employees and agents, from any and all liability, claims, or demands for personal injury, sickness or death, of any nature whatsoever, which may be incurred while participating in the Nordic Walk Now programs, classes, training sessions, workshops, or any other event, instruction, and/or walks organized by Nordic Walk Now or Suzanne M. Nottingham, Inc. Furthermore, I hereby assume all risk of personal injury, sickness, death, damages and expenses as a result of participation in Nordic Walking classes, training sessions, workshops, programs or any other event, instruction, and/or walks organized by Nordic Walk Now or Suzanne M. Nottingham, Inc.

Also, I understand that I should consult with a medical professional before undertaking any exercise program, and further state that if I fail to do so such failure on my part is not in any way attributable to Suzanne M. Nottingham, Inc., Nordic Walk Now, its directors, instructors, trainers or employees.



The undersigned further hereby agrees to hold harmless and to indemnify Suzanne M. Nottingham, Inc., Nordic Walk Now, its directors, employees and agents for any liability sustained, including expenses incurred attendant thereto, as the result of any negligent or intentional acts by myself while participating in the activities organized by Nordic Walk Now or Suzanne M. Nottingham, Inc.
